Please describe your puppy’s living environment and home life! Please include things like how many levels your home is, if you're in an apartment or house, what your neighborhood is like, and who lives in your home.

Where will your puppy spend most of his/her free time? (what rooms of the house? Who else spends a lot of time there? What types of activities happen there? What are you planning to do to make it dog friendly?)

What are some activities you'd like to bring your dog along for?

During times you cannot supervise them or will be away from your house, will your puppy have a playpen or other contained area to spend some unsupervised time or do you prefer them to only be in a kennel?

Where will your puppy sleep/where will the kennel be kept?

Will your puppy be allowed on furniture like chairs and couches?

Will your puppy be allowed on human beds during leisure times? (watching tv, napping, reading, snuggling, etc.)

If you have a yard, is it fenced? Is there a specific area you'd like for them to potty in? Will they need to potty on a leash when they arrive home?

Where will you feed your puppy? (in a crate, in the main area of the home, etc.)

How experienced of a dog owner are you? If you have had a puppy in the past, who trained it?

What does a typical day in your life look like, and how does your puppy fit into that? What are your weekends like?

Do you have any other special requests or anything else you would like our trainers to be aware of about your specific lifestyle or training needs?

I understand that when it's time for my puppy to come home, they will have an adjustment period that may result in crying in their crates, disobedience, and more. We will happily help you work through this!

I understand that in the event of sickness or injury during the duration of my puppy's stay in training, cost will be covered by training company.

I understand that changing my puppy's food directly after they arrive home may result in upset stomach or training regression. We do not suggest this.

I understand that even after my puppy comes home, they will need to continue their training. We are laying the groundwork, but their ultimate success is dependent on the actions you do or do not take after their homecoming.
